Media Consent Form

The Corner Brook Rapids Swim Club promotes involvement in activities by posting swimmer photos, audio, and/or video of swimmers on our website, social media platforms (e.g., Twitter) and/or newsletters, presentations, brochures or advertisements. Activities and achievements of our club may also be promoted on the Corner Brook Rapids Swim Club website (www.cbrapids.com), social media platforms, or via other promotional opportunities. Occasionally, media may be present in our club to videotape, photograph, record, and/or interview swimmers involved in swim club-related activities. Please note; activities considered ‘in swimming’ include, but are not limited to; pictures and video of activities that occur in the swimming pool environment; outside and the physical building. Please indicate your wishes regarding your swimmer’s involvement in the promotional activities noted above by completing the form below and returning it to your swimmer’s coach or one of the Corner Brook Rapids Executive Members. Swimmers names or images will not be used without permission from the parent/guardian.
